「もやもや病」の診断基準の曖昧さについて ウィリス動脈輪閉塞症（もやもや病）は、我が国で発見され、かつ最も多くの患者を有していますが、未だその原因は不明です．厚生労働省 特定疾患ウィリス動脈輪閉塞症調査研究班によりその診断基準が定められていますが、その診断基準の中の脳血管撮影所見で「頭蓋内内頚動脈終末 部、前及び中大脳動脈近位部に狭窄または閉塞がみられる」の項目が曖昧です．つまり、（１）内頚動脈終末部、（２）前大脳動脈近位部、（３）中 大脳動脈近位部の３部位のうち、すべてに狭窄または閉塞を認める必要があるのか、または一つの部位でもその診断基準を満たすのか、が曖昧である と思います．このことは、ウィリス動脈輪閉塞症調査研究班の研究報告書にある英文の診断基準では、 　　　Stenosis or occlusion at the terminal portion of the internal carotid artery and at the proximal portion of the anterior and middle cerebral arteries. となっていますが、同じ研究班から出された別の論文では、 　　　Stenosis or occlusion at the terminal portion of the internal carotid artery and/or at the proximal portion of the anterior and/or middle cerebral arteries. となっていることでも分かります．また、研究班のメンバー施設からの論文では、内頚動脈終末部の狭窄・閉塞性変化は、必要であるととれる英訳になっています． 　　　Stenosis or occlusion is observed at the terminal portion of the intracranial internal carotid artery and/or at the proximal portion of the anterior cerebral artery and/or middle cerebral artery. 　　　Stenosis or occlusion of the internal carotid artery at the level of the terminal portion together with abnormalities of the anterior and middle cerebral arteries. これらの点は、ウィリス動脈輪閉塞症の確実症例と疑い症例の区別にも重要な点であり、確定診断に必要な狭窄・閉塞性変化の部位を明確に規 定する必要があると考えます． 以上の意見が、1999年に脳神経外科の専門誌に掲載されましたが、残念ながら、この点に関しての変更・改善は今のところありません． 参考文献 1. 小宮山雅樹：ウィリス動脈輪閉塞症の診断基準について．（読者からの手紙）．脳神経外科 27: 481, 1999
